Our community cardiology and respiratory service provides assessments and diagnostic tests for people who may have problems with their heart, blood pressure or breathing.
The service is consultant-led and based at various clinics in our local communities, allowing patients to be seen and treated closer to home instead of in hospital. Each clinic offers a tailored range of services, so you will be referred to the clinic that is most appropriate for your symptoms and closest to your home.
Conditions and treatments
We offer an efficient and effective service to treat a range of heart-related symptoms and conditions including:
- chest pain
- shortness of breath, at rest or on exertion
- arrhythmia (palpitations or slow/fast heart beat) and specifically atrial fibrillation (abnormal heart rhythm)
- collapsing or fainting episodes
- general problems with the heart
- uncontrolled hypertension (high blood pressure)
- heart failure (including swelling of ankles, breathlessness at night)
In addition, our community clinics can treat a range of respiratory symptoms and conditions, including:
- COPD: for optimisation and long-term management and support
- shortness of breath, chronic cough or frequent chest infection with normal chest x-ray
- asthma – for disease management and optimisation
- bronchiectasis – for airways clearance and medical review
- pulmonary rehabilitation – in patients with known respiratory disease with breathlessness on exertion
- chronic resting hypoxia (SpO2 <92%) that may require or have home oxygen
We offer the following diagnostic tests:
- echocardiogram
- electrocardiogram (ECG)
- 24-hour ECG
- 24-hour blood pressure monitoring
- spirometry (for respiratory patients only)

Before your appointment
We will book an appointment for you within two working days of receiving a referral letter from your GP. The date and time of your appointment will be sent to you via a letter in the post. If there is any doubt that the letter will not arrive in good time you will be contacted by phone.
Before your appointment you may need to have tests carried out on your heart. These tests will be arranged by our community clinics. They will normally be carried out at the same clinic as your specialist appointment, although they may not be carried out on the same day of your appointment. Some tests will need to be done before the day of your appointment, as they will involve wearing a heart monitor for anywhere between 24 hours and seven days.
These tests may be completed a few days before your appointment or may be conducted on the same day. This is so the results can be discussed during your consultation.
Before attending for any test you are able to eat and drink as normal. You may want to bring a snack if you are going to be at the clinic for a while. There is drinking water available.
If you are being seen in the chest pain or shortness of breath clinic you are advised to wear flat, fully covered shoes or trainers as you may be asked to perform a treadmill test, if you are able to do so.
It is very important to bring all your medications to your appointment. Please remember to check your appointment letter for anything specific you may have been asked to bring with you. In addition, it would be helpful if you could bring the following:
- full address and telephone number(s)
- appointment card and appointment letter
- GP’s name and address
- a list of questions you may wish to ask during your visit
During your appointment
Upon arrival please visit the main reception. We will make every effort to ensure you are seen on time but sometimes a delay is unavoidable. Our team will ensure you are kept up to date about any delays.
You are welcome to ask for a chaperone and we will endeavour to provide one for you.
Before your consultation you will have an ECG and your height, weight and blood pressure recorded. You will see either a consultant or specialist nurse during your appointment.
The majority of patients can expect to be at the clinic for less than an hour. If you are attending some clinics for specific conditions you may need to have a number of tests performed on the same day as your consultation which may result in an attendance of up to three hours. You will be notified in advance if this is the case.
You will see a consultant or a specialist nurse at your appointment. They will discuss the results of any tests you may have had as well as their findings and recommendations. You may be referred to hospital for further tests or your condition will be further managed in the community. The consultant or specialist nurse will discuss next steps with you.
After your appointment
Following your appointment, the consultant or specialist nurse will make a diagnosis and will send recommendations and advice to your GP in a letter. You will receive a copy of this letter.
Alternatively, in some cases a referral will be made for you to be seen in one of our specialist hospital-based cardiology clinics.
Refer to this service
GPs
If you are concerned that your patients are experiencing cardiac and respiratory symptoms, you may refer them to our service and we will give them an appointment at the most appropriate clinic location.
This is a one-stop service, so we ask that when you refer patients to us, you include the following results:
- chest x-ray
- diagnostic spirometry
- blood tests including urea and electrolytes (U&Es), full blood count (FBC), UFTs, thyroid function (TFT), glucose and cholesterol
- for referrals into the heart failure service, brain natriuretic peptide test (BNP)
In addition to our community clinics, we offer a direct GP referral service for rapid access to cardiac and respiratory diagnostic testing with reporting provided by specialist consultants.

GP advice
Our community cardiology and respiratory service provides email and phone-based advice for GPs. The advice email is monitored by consultants every Monday through Friday, and GPs can expect a response within 24 hours. The telephone helpline is a resource that enables patients and GPs to quickly access specialist advice. Advice for GPs is provided by our consultants, and urgent patients can be slotted in quickly if indicated via telephone.

Additional information for GPs
Many patients can have their care managed entirely within community clinics, but some conditions, investigations and procedures are best managed in hospital or through other community services. These include:
- device (ICD) clinics
- adult congenital heart disease at the Royal Brompton & Harefield NHS Foundation Trust
- hyperlipidaemia (lipids)
- inherited cardiac conditions
- Pacing checks
- Patients with an abnormal chest x-ray and possible lung cancer (secondary care)
- diagnosis or management for patients with primary restrictive lung disease (secondary care)
- respiratory physiotherapy or nursing for patients without a chronic respiratory disease (general community rehab or nursing service)
- investigations for respiratory muscle weakness (secondary care)
- sleep service (secondary care at Charing Cross Hospital)
- long term home nebuliser service (secondary care or district nursing)
If you wish your patient to be considered for one the following tests or procedures, please refer them into the community service. The consultant will see and assess your patient and then make the appropriate referrals. More complex tests include:
- Invasive cardiology procedures, including diagnostic coronary angiography, coronary PCI (stents), right heart catheter assessment
- Stress imaging (stress echocardiography or stress perfusion MRI)
- Transoesophageal echocardiography to investigate valvular heart disease
- Tilt table testing for patients having recurrent collapses or syncope
Additional information
In addition to the services above, we also offer services for heart failure, including heart and shortness of breath clinics and a home visiting service for people with heart failure. We also offer a variety of specialist respiratory clinics.
When we see you in our community clinics, we may find that we require further specialist investigations or procedures that can only be performed in a hospital setting in order to help treat you more effectively. In those cases, the consultant overseeing your care in the community will refer you as appropriate.
