The Advanced Kidney Care (AKC) team offers advice and support across west London to patients with advanced kidney disease who are approaching end-stage renal disease.

The team consists of nurse specialists and advanced practitioners who work in partnerships with consultant nephrologists. Their services include clinical assessment, education and psychological support to aid decision making with regard to their treatment options (transplantation, dialysis, conservative management).

Clinics are located at Hammersmith, Charing Cross, St Mary’s, Ashford, Watford, West Middlesex University Hopsital and London Northwest University Hospital (Central Middlesex, Northwick Park and Ealing).

Referral pathway

There are two routes of referral into the Advanced Kidney care clinics (AKCC): internal or external. Internal referrals are made where a patient transfers from the general kidney care clinic at the recommendation of the Consultant or Nurse Specialist. External referrals where a patient moves relocates should be made via the NHS Electronic referral service (ERS) to the ‘NWL Kidney Care’ portal. The patient will be seen in a general nephrology clinic initially and transferred into the AKCC or specialist clinic as appropriate.

Patients with progressive kidney disease (stage 3 – 4) or requiring specialist management, such as immunosuppression, are managed in the general or specialist kidney care service. When their Kidney disease approaches stage 5 requiring kidney transplantation, renal replacement therapy or, in some cases, conservative management, they are transferred to the Advanced Kidney Care clinic (AKCC). The AKCC team which consists of a consultant nephrologist, nurse specialist, dietician and counselling services will work closely with the patient to provide education and support to aid decision making in deciding their chosen pathway.
