My transplant is not working well - what next? 

Many patients will need more than one transplant in their lives for health and well-being. We have a bridging clinic within the kidney transplant clinic at Hammersmith Hospital led by doctors and clinical nurse specialists who are trained in: 

• explaining how well your kidney transplant is working for you 
• helping to improve your kidney transplant, anaemia and heart health
• helping you to make the best choices for care going forwards 

We want to protect your kidney transplant function for as long as possible to remain well. When it is no longer doing that, we will share a plan with you for another kidney transplant, dialysis or supportive care.
