If you experience or observe extraordinary and compassionate care by a nurse or midwife at the Trust, please use the online form below to nominate them for a Daisy Award.

Patients, their families or carers can nominate nurses and midwives at our Trust, who have shown outstanding clinical skill and compassionate, for a ‘DAISY Award for Exceptional Nurses’. 

Four awards will be presented every two months with the ‘winners’ receiving a Healer’s Touch sculpture, a copy of the nomination and their name entered onto the Healer's Touch website.

Why nominate?
This is great way to formally express your gratitude to a nurse or midwife for exceptional care. By nominating them, you are highlighting their good work to others at the trust and giving them the chance to win an internationally recognised award.

What happens when I nominate someone?
Every nomination is reviewed by the trust’s reward and recognition council who will select the ‘winning’ nomination against the DAISY Award criteria. Every two months, four awards will be given to staff at the trust, in a special ceremony led by a senior member of staff in the winners’ place of work. They will also receive a Healer’s Touch sculpture, a copy of the nomination and their name entered onto the Daisy Foundation website: daisyfoundation.org/daisy-award/honorees

What is the DAISY Award?
The DAISY (Diseases Attacking the Immune System) Award was established in America by the DAISY Foundation in memory of J. Patrick Barnes who died aged 33 of an auto-immune disease. The Barnes family was so deeply appreciative of the care and compassion Patrick received from the nurses who cared for him, they created an international award to say “thank you” to nurses and midwives everywhere. 

How do I nominate someone?
You can nominate a nurse or midwife for an award by completing a nomination form below. You can also ask for a paper nomination form at one of our main reception desks (at all of our hospital sites). Please return your completed paper form to one of our friendly receptionists.

Who can nominate someone?
Patients, or their families, friends or carers.

Can I make a nomination anonymously?
Yes, but we would encourage you to leave your details so we can let you know the outcome of your nomination.

How will I know if my nominee is successful?
If it is possible, we will invite you to attend the award presentation, if not, we will e-mail you to let you know if your nominee was successful.
