Please use NHS e-Referrals to refer a patient to our kidney and transplant services, which can be accessed via general nephrology clinics on NHS e-Referrals.
All patients with chronic kidney dysfunction should be referred according to the Royal College of General Practitioners guidelines.
Patients may be transferred to specialist clinics such as the vasculitis, vascular access or predialysis clinics as required.
Patients with specialist conditions may be transferred by their nephrologist to specialist clinics such as the vasculitis, predialysis or lupus clinics as required. Appropriate GP referrals are assigned to the Lupus Centre after initial assessment.
We offer an email advice service to support GPs and practice staff as they manage their patients with chronic kidney disease within primary care.

Secondary care referrals
Referrals for transplantation are primarily made internally, but other renal centres can also refer patients if they are not a transplant centre or if a patient requires a high-risk transplant.
