Paediatric diabetes homecare team

Our team provides a dedicated and dynamic service to all children with diabetes who attend the Trust. We support and educate all our patients and their families at home to manage their long-term condition and believe they are the centre of our team. We also support and educate all professionals involved in the care of the diabetic children at school and in the hospital. 

We aim to lead the way in advanced diabetes practice. We have established the Continuous Glucose Monitoring System clinics which proved to be a powerful tool in better understanding individual diabetics. We will also be starting the pump therapy as of the middle of 2011. We provide a range of education and support services for our patients, families and staff including facilitated parents’ groups, carbohydrate counting workshops for families, educational and peer-building afternoons and in-house staff training days. 

We work very closely with the Chelsea and Westminster paediatric diabetes team and also with the trust adult diabetes team through dedicated young adults diabetes clinic.
