What you need to bring

You will need to bring the following to your appointment:
• Your full address, including postcode
• Your daytime and evening contact telephone numbers
• Your NHS number
• Your next of kin's name and full address, including postcode
• Your GP's name and full address, including postcode
• Your appointment card or letter
• Any medication that you are currently taking (or a list of it)

You may also need:
• Money to pay for prescriptions, or an exemption card if you are entitled to free prescriptions
