Throughout your stay we will discuss with you and your family the arrangements for your care once you leave the hospital.
Your leaving time
On the day you are leaving hospital you will be asked to vacate your bed by 10.00 so that it can be prepared for another patient. You may then be asked to wait in the patient transport lounge.
Transport arrangements
You will be expected to make your own transport arrangements for going home unless your consultant feels that there are special reasons why you need hospital transport. If you are on a low income or receive benefits you could get help with your transport costs.
Aftercare
After you leave hospital your consultant will write to your GP to let them know about your care in hospital. If you need medication to continue your treatment at home, a member of staff will give this to you before you go. Please make sure you know how to take it before you leave. Your GP will give further instructions if necessary.

• Have you given the ward clerk your forwarding address?
• Have you been given an outpatient appointment if needed?
• Have you given back any hospital property?
• Have you got money to pay for your transport home, gas or electric meters, etc?
• Have you got your house keys?
• Have you told your relatives, friends or neighbours that you are coming home?
• Have you received your discharge letter?
• Does the ward have details of your GP?
• Do you have a supply of medication to last until you see your GP?
• Have you requested the return of any money or valuables?
