Palliative care service

The palliative care team is available for advice on symptom assessment and management, complex discharge planning, advanced care planning, assistance in difficult discussions around end-of-life issues and terminal care management in adult inpatients with a life-limiting illness. There are consultants and clinical nurse specialists on each hospital site, with a lead clinician (Dr Katie Urch) and nursing team leader (Diana Howard) across the Trust. 

The team’s approach to care is holistic and we work closely with our psychology, chaplaincy, welfare benefits, social service, community and the Macmillan rehabilitation team to ensure that patient-centred care is our focus. 

Specialist palliative care advice is available 24 hours/seven days a week. 

Referrals
The team takes referrals from any staff member, as well as patients and their carers. Referrals can be made over the telephone or in person. If necessary a message can be left on the answer machine on either site and a team member will contact you within 24 hours. 

More urgent referrals can be made by bleeping the CNS on each site (Monday to Friday, 09.00 to 17.00). 

The team is happy to see or discuss any patient who has a life-limiting illness and is an inpatient or attending outpatient clinics.
