Elderly medicine

The department of Medicine for the Elderly specializes in the care of frail older people with complex problems such as delirium, dementia, difficulty walking and falls.  Inpatient services focus on acutely ill elderly people.  The rehabilitation ward provides specialist rehabilitation planning for patients referred for rehabilitation.  Outpatient services include general and specialist clinics, memory, falls and Parkinson’s which are run on site and in the community.

Services are run at Hammersmith Hospital with a strong focus on comprehensive elderly assessment, multidisciplinary teamwork and discharge planning.  The department works closely with a range of other internal and external specialties and care agencies

Hammersmith Hospital offers the following clinical services:

Inpatients – acute, rehabilitation and liaison

  • Acute consultant-led comprehensive elderly assessment service (OPAL service)
  • Acute inpatient medicine for the elderly service
  • Rehabilitation referrals service
  • General inpatient consultation service

Outpatients and ambulatory

  • Specialist multidisciplinary falls clinic in partnership with community falls intervention service
  • Medicine for the elderly outpatients clinic (comprehensive elderly assessment)
