For GP referrals to the prosthetic service, complete a referral form online or contact the centre manager to request a referral form.

Primary patients should be seen within two weeks.

For an emergency referral, call before 10.00 on the day.

For urgent referrals, call the centre manager on the number given above.

Prosthetics (head and neck/maxillofacial prosthetics and technology)
Patients are referred by healthcare professionals: oral and maxillofacial surgery, ophthalmology, head & neck oncology, neurosurgery, plastic surgery, cranio-facial surgery, ear nose and throat, breast care services and from GPs.
