Referrals to Hammersmith Hospital

Referrals are via Choose and Book or by fax to 020 8749 6973.
Faxed referrals are preferable to referrals by post.

Urgent referrals should be faxed and clearly marked URGENT. Preliminary hormone and sperm analysis tests should be completed in primary care.   

Out-of-area patients and those with existing children, sometimes on either the male or female side according to PCT criteria, should only be referred as private patients.

Patients over 39 may not be eligible for NHS treatment by the time funding is available, and anyone with a BMI over 30 must reduce their weight before being referred.

If either partner has been sterilised, NHS treatment cannot be offered.
Please include all the above information in the referral letter.

Referrals to Queen Charlotte’s and Chelsea Hospital

Referrals are via Choose and Book or by fax to 020 8749 6973.

Referrals to St Mary’s Hospital

Referrals can be made via Choose and Book to either of the two multidisciplinary fertility clinics for patients wishing to conceive. For patients not wishing to conceive referrals can be made directly to the reproductive endocrine clinic.
