Referrals to Charing Cross Hospital

All new patients for lifestyle changes, pharmacotherapy or surgery should be referred to the Imperial Weight Centre via Choose and Book.

Paper/fax referrals (preferably using our GP referral form available from www.imperialweightcentre.co.uk) should be sent to Imperial Weight Centre, or faxed to
020 3313 0673.
