GPs are the first point of contact for most health services, so it is important for you to be registered with one. If you are a local resident of Hammersmith and Fulham you can register with us today.
‘The Hammersmith and Fulham Centres for Health is a first class General Practice based in two leading London Hospitals. Each site provides services to local residents of Hammersmith and Fulham. The two sites are at Hammersmith Hospital and Charing Cross Hospital. You only need to register at one site and can be seen at either site for your convenience. You will experience excellent care provided by experienced and caring GPs and have access to modern facilities.
As each practice is based within a hospital that is affiliated to Imperial College, you will have access to world class specialists and the latest technological advances and investigations. Our aspiration is to become the best General Practice in the UK and hence you will receive the best care and service possible. We are a teaching practice so must maintain high standards of care and performance.
To help us to continuously improve our services and seek your input into new ideas we are actively inviting our Practice patients to participate in a development and feedback group.

Opening times
We are open daily from 8am-8pm (including weekends but excluding bank holidays) and offer longer than average appointments of 15 minutes so we are able to listen and understand your problems and decide what treatment is best for you. We offer double appointments and family appointments.
The GP Practice offers a full range of GP and nursing services and minor injury/illness care at the two sites seven days a week.

We offer a range of excellent nurse/GP services at the Hammersmith and Fulham GP practice, including:
• Chronic disease management e.g. asthma checks, heart disease monitoring, diabetes care
• Women’s health
• Contraceptive services
• Sexual health
• Vaccination/immunisation including travel advice
• Child health services
• Stop smoking services
• Lifestyle advice, such as sensible drinking advice
Our practice nurse treats a wide range of common conditions and is qualified to prescribe from a range of medicines. Our practice nurse Augustina Eyeson is very experienced in chronic disease management.
The Practice operates alongside the Urgent Care Centres adjacent to the emergency units at Hammersmith Hospital and Charing Cross Hospital.

Before you can make an appointment to use our GP services, you need to be registered with us.
How to register
We accept new patients who live in the borough of Hammersmith and Fulham.
To register, you need to pick up a registration form GMS1 from this link: GP Registration Form or from reception at either the Fulham or Hammersmith Centre for Health.
Once you have completed your registration form, you need to return it to the reception at either centre along with your medical card, if you have it, proof of your address, e.g. a utility bill, tenancy agreement and photo ID e.g. passport or driving licence.
When you register we will offer you an appointment with our practice nurse, Augustina Eyeson, for a ‘new patient check-up’. This helps to make sure that our records are accurate and that all your routine health checks, such as blood pressure or cholesterol tests, are up to date.
Once your registration is completed, your medical records will be transferred to us and a new medical card will be sent to your home address. You don’t need to inform your existing GP that you are moving.
Making an appointment
Once you are registered with us, you can make an appointment to see a GP or nurse at either centre by calling 020 8893 0384 or visiting reception at either of the centres between 08.00 and 20.00.
We offer appointments from 08.00 to 20.00, seven days a week. If you have an urgent problem, we aim to see you on the day you call. If your condition isn’t urgent, you can expect to see a GP within two working days, though you may have to wait longer if you want to see a particular GP. You can expect to see a nurse within one working day.
If you don’t need an appointment within two working days, you can book up to eight weeks in advance and with the GP of your choice.
