Interprovider referrals

From January 2008 it has been mandatory to complete the minimum data set (MDS) for referrals between hospitals for patients on an
18-week referral to treatment (RTT) pathway.

Interprovider referrals to Imperial College Healthcare NHS Trust
Please ensure the MDS for 18 weeks is completed for all interprovider transfers (IPTs) and sent with the referral letter. Alternatively it can be sent to our interprovider coordinator electronically within 48 hours of the decision to refer.

The NHS 18 weeks team has produced a minimum data set template (PDF, requires Adobe Acrobat Reader, free download) to assist referring trusts.
